Robert C. Macauley is Cambia Health Foundation Endowed Chair in Pediatric Palliative Care at Oregon Health and Science University. Ethics in Palliative Care is a book only Dr. Robert Macauley could have written. Macauley is a pediatrician, medical ethicist, palliative medicine specialist, and Episcopal priest. He is one of very few physicians with career-long experience in both clinical ethics and palliative care. He formerly directed the Clinical Ethics service at the University of Vermont Medical Center and currently holds an endowed chair in pediatric palliative care at Oregon Health and Science University. He is a physician extraordinaire.Macauley’s book covers major topics in clinical ethics and palliative medicine. It combines historical accounts that trace the evolution of practices with balanced analyses of controversial topics and clinical bedside strategies gleaned from personal experience. His accounts are at once scholarly and practical. His writing is clear with summary points concluding each chapter and recent relevant references.Particular strengths of the book include Macauley’s thoughtful and balanced discussions of advance care planning, forgoing life-sustaining therapy, physician-assisted death, palliative sedation to unconsciousness, the principle of double effect, and requests for non-beneficial therapy. His in-depth discussions of pediatric ethics and palliative care are especially noteworthy because these topics are covered only superficially in many books. Here, Macauley carefully distinguishes pediatric from adult issues, and discusses prenatal, neonatal, childhood, and adolescent issues in separate chapters. His discourse on the intersection of clinical ethics and palliative care is the finest I have read anywhere. His brain death section has thoughtful advice for physicians to sensitively manage the emotionally charged situation when a patient’s family refuses to accept the diagnosis.
